Those who are 65 and older have a specific window where they can apply for Original Medicare Parts A and B without any penalty charges. Excluding those on disability or those who have preexisting conditions, everyone has an Initial Enrollment Period that starts three months before they turn 65, includes the month of their 65th birthday, and ends three months later. But that’s only the first step…

Most American citizens get Medicare Part A for free if they have worked at least 10 years (40 quarters) and paid taxes during that time. Someone who has worked off the books or with little work history will have to pay a premium for their Part A. The Medicare Part A late enrollment penalty is a 10% higher premium for twice the number of years that you were eligible, but didn’t sign up for Medicare Part A.

In comparison, everyone is required to pay a premium for Medicare Part B. But if the deadline to sign up is missed and the individual doesn’t have other health insurance, they could face a late enrollment penalty. The Medicare Part B late enrollment penalty is a 10% higher premium for each full 12-month period that you were eligible, but didn’t enroll in Medicare Part B.

Once approved for Medicare Part A and Part B, you are required to enroll in a Part D Prescription Drug Plan (PDP). Most of the time, the Initial Enrollment Period for Medicare Part D will take place simultaneously during the Initial Enrollment Period for Part A and Part B. Applying outside of this window will result in a late enrollment fee, with some exceptions. For example, someone who is in the Extra Help (Low-Income Subsidy) program won’t have to pay a late enrollment penalty for Medicare Part D.
